Dia-Care™ Shipping Policy
In order to best serve our patients, we ask that you please check your delivered order for accuracy. A "Proof of Delivery" form will be included with your shipment. Please sign and return this form within 5 days of receiving your supplies.
If you have changed to an HMO, kindly return all supplies unopened within 30 days. To expedite this process, please provide a copy of your insurance card and/or Medicare card as required for our records. Simply use the convenient postage paid envelope enclosed.
